Itemized Medical Bill
An itemized medical bill is a detailed hospital bill listing each charge separately, including procedure codes (CPT), revenue codes, medications, facility fees, and professional fees. It replaces the summary bill that hospitals send by default.
Why it matters
Summary bills show a total amount but do not reveal how that total was calculated. An itemized bill allows patients to identify errors, unbundled charges, duplicate entries, and incorrect coding. Requesting an itemized bill is the first step in any medical billing review.
How to request one
Call the hospital billing department and ask for a coded itemized bill. Hospitals are required to provide this document upon request.
